Routine Vaccine Schedule, 2017

  • Birth=HBV
  • 2 months=Pediarix HBV PCV Rotavirus
  • 4 months=Pediarix HBV PCV Rotavirus
  • 6 months=Infanrix PCV Rotavirus
  • 12 months=MMR PCV
  • 12-15 months=Varivax HIB
  • 18 mo=DTaP
  • 12-24 months=HAV
  • 18-36 months=HAV
  • 4-6 years=Kindrix MMR Varivax
  • 11-12 years=Tdap
  • 11-12=MCV
  • 16-17=MCV
  • 11-26=HPV series
  • >6 months=Fluzone or Flumist every fall
  • HBV = hepatitis B
  • Pediarix = DTaP + HBV + IPV
  • HIB = hemophilus influenzae type B
  • PCV = pneumococcal conjugate (Prevnar)
  • Rotavirus = Rotateq (3 dose – 2, 4, 6 months)
  • Infanrix (DTaP) = diptheria + tetanus + pertussis (whooping cough)
  • Varivax = varicella (chickenpox)
  • MMR = measles + mumps + rubella
  • HAV = hepatitis A
  • Kinrix = DTaP + IPV (for over age 4)
  • Tdap = tetanus + diptheriea + pertussis (for over age 7)
  • MCV = meningococcal conjugate
  • HPV = human papillomavirus (cervical cancer virus)
  • Influenza vaccine = Fluzone shot or Flumist nasal
  • IPV = inactivated polio
