Routine Vaccine Schedule
- Birth – HBV
- 2 months – Pediarix – HIB – PCV – Rotavirus
- 4 months – Pediarix – HIB – PCV – Rotavirus
- 6 months – Pediarix – PCV
- 12 months – MMR – PCV – Hep A
- 15 mo – Varivax – HIB
- 18 mo – DTaP – Hep A
- 4-5 years – Kinrix – MMR – Varivax
- 11-12 years – Tdap – MCV – Start HPV Series
- 16-17 years – MCV
- Influenza- every fall for age 6+ months
- For travel – Typhoid
Pediarix = DTaP + HBV + IPV
Rotavirus = Rotarix (2 dose – 2, 4 months)
Hib= haemophilus influenza type B
PCV= pneumococcal conjugate
DTaP = diphtheria + tetanus + pertussis (whooping cough)
Kinrix = DTaP + IPV (for over age 4)
Varivax= chicken pox
MMR= measles + mumps + rubella
Tdap = tetanus + diphtheria + pertussis (for over age 7)
MCV= meningococcal vaccine, types A/C/Y/W-135
HPV= human papillomavirus
