Birth=HBV

1 or 2 months=HBV

2 months=Pentacel-PCV-Rotavirus

4 months=Pentacel-PCV-Rotavirus

6 months=Pentacel-PCV-Rotavirus

6 - 12 months=HBV

12 months=Varicella-PCV

18-59 months=PCV13 if not yet given

12-15 months=MMR-HIB

18 mo=DTaP

12-24 months=HEP A

18-36 months=HEP A

4-6 years=DTaP-IPV-MMR-Varicella

11-12 years=Tdap-Meningococcal

11-26 years (girls)=HPV

>6 months=Fluvax or Flumist every fall

HBV = hepatitis B

Pentacel = DTaP + HIB + IPV

DTaP = diphtheria + tetanus + acellular pertussis (whooping cough)

HIB = hemophilus influenzae type B

IPV = inactivated polio

PCV = pneumococcal conjugate

Varicella = chickenpox

MMR = measles + mumps + rubella (German measles)

HEP A= hepatitis A

Tdap = tetanus + diphtheria + acellular pertussis for

age 11 and up

Fluvax = influenza shot Flumist = influenza nasal

HPV = human papillomavirus (cervical cancer virus)
